Important Details

⚠️ Acute illnesses require a GP visit. Sick notes cannot be requested here. For long-term illnesses with prior sick note only.
  • This service is for existing patients only.
  • This form is for patients with long term illness, covid etc.
  • You must already have a sick note or social welfare certificate and require an extension.
  • Please note: Only Social Welfare Certificates are free.
  • Sick Note Fee (all patients): €20 or €35
  • A minimum of 48 working hours’ notice is required for processing.
